Any patient added to Connect and Coach has the option to complete a pre-assessment survey through a secure online patient portal.  If you have the patient’s e-mail address when you create their record  in Connect and Coach, you can choose to email a link to the pre-assessment survey in advance of your appointment. You can also send a pre-assessment survey for a patient who was previously entered into Connect and Coach.

To add a patient and email a pre-assessment survey:

initiate PSA

Create a New Patient
  1. At a minimum, add First Name, Last Name and Email.
  2. Select a PSA from the Survey (PSA) drop down menu.
  3. Click Create a New Patient

While Email is not solely a required field, entering an e-mail address for a patient is required to send the pre-assessment survey. When a patient completes a pre-assessment, a number of Connect and Coach fields are automatically populated within the initial assessment and the overall record with the information the patient provides.  You will be prompted to confirm that you wish to send the PSA and a welcome email.

Send Welcome E-Mail

To confirm that you want to send a welcome e-mail, click Yes, Send Welcome Email.

Once you have confirmed your intention to send a welcome e-mail, Connect and Coach sends an e-mail to the patient at the e-mail address you provided. The e-mail introduces the Connect and Coach platform, and asks the patient to follow a link to the secure patient portal website. Here, the patient will log in to Connect and Coach for the first time using the login credentials provided in the welcome e-mail.

After changing the account password, the patient will be directed to complete the pre-assessment survey.  The information the patient provides in response to this survey populates a variety of patient information fields in the initial assessment and within the overall Connect and Coach patient record.

E-mailing a Pre-Assessment Survey to an Established Patient

To e-mail a pre-assessment survey to a patient who is already in Connect and Coach, you must first select the patient.

Select a Patient
  1. Click the Patients tab.
  2. To locate a patient, you can search by First Name and/or Last Name. The smart search will populate all records with matching information until you narrow down the search to your patient.
  3. Click on View Patient Details to access the patients’ record

Once you are in the patient’s record:
