Signing Consents

PHRQL provides a way for you to store electronically signed consent and waiver forms for your patients.  You can gather consent signatures in one of two ways.  1.) the customer can sign the consents before they attend a session while completing the pre-assessment questionnaire (PSA) or,  2.) you can have them sign the consents in your office from the General Information tab.

The patient must sign the consents before answering questions on the PSA.  The consents they sign will appear like this screenshot.  They also have the option to download and print a signed copy. Note: Consents are site specific.

If the patient did not sign the consents through the PSA you can have them sign electronically in your office.  The consent files are stored in the General Information tab under the Customer Releases and Consent Forms section.

Sign Consents
  1. Click the General Information tab.
  2. Navigate to Customer Releases and Consent Forms and click on the Edit pencil.
  3. Click on the name of the consent, this is a live link that will load the consent form.
  4. Patient types their name in the Signature field
  5. Save Changes

Chapter 3. Consent on file

Sometimes a consent has already been signed in paper form and is on file in the office.  In this case you have 2 options, you can request the patient sign again electronically OR you can choose “on file” in a couple of places where consent forms can be signed.

Option 1 – In the Assessment form:

After navigating to the Customer Releases and Consent Forms section you can choose “on file” from the drop down menu for Method Accepted.
